四、申請(qǐng)材料

IV. Application Documents

申請(qǐng)者須提交以下材料（一式兩份）：

Applicants must submit their documents according to the following instructions (all in duplicate):

1．《孔子學(xué)院獎(jiǎng)學(xué)金申請(qǐng)表》。

1. Confucius Institute Scholarship Application Form;

2．護(hù)照照片頁(yè)復(fù)印件。

2. Photocopy of passport photo page;

3．申請(qǐng)陳述。

3. Application statement:

申請(qǐng)者須用中文介紹漢語(yǔ)學(xué)習(xí)背景、來(lái)華學(xué)習(xí)計(jì)劃及目標(biāo)等。其中，四周研修生獎(jiǎng)學(xué)金申請(qǐng)者不少于200字；一學(xué)年研修生獎(jiǎng)學(xué)金申請(qǐng)者不少于800字；漢語(yǔ)國(guó)際教育專業(yè)碩士獎(jiǎng)學(xué)金申請(qǐng)者不少于1500字。

indicating the applicant's Chinese learning background and study plan in China. It should be no less than 200 words for the Four-Week Study Scholarship applicants, no less than 800 words for the One-Academic Study Scholarship applicants and no less than 1,500 words for the Master's Degree Scholarship applicants;

4．新HSK成績(jī)報(bào)告復(fù)印件（如申請(qǐng)者尚未取得新HSK成績(jī)報(bào)告，可暫時(shí)提交新HSK準(zhǔn)考證號(hào)及其他漢語(yǔ)水平證明材料，但須在入學(xué)時(shí)提交正式的新HSK成績(jī)報(bào)告）。

4. A copy of the new HSK result report (for those who have not been given a result report, the admission ticket number and other Chinese proficiency certificates are required, but must submit the new HSK result report when enroll);

5．最高學(xué)歷和成績(jī)證明。一學(xué)年研修生和漢語(yǔ)國(guó)際教育專業(yè)碩士獎(jiǎng)學(xué)金申請(qǐng)者須提交經(jīng)過(guò)公證的最高學(xué)歷證明；漢語(yǔ)國(guó)際教育專業(yè)碩士須同時(shí)提交學(xué)校的成績(jī)證明。

5. Diploma and transcripts. Applicants for One-Academic Year and Master's Degrees Scholarships should submit notarized diplomas; and the latter are also required to submit transcripts issued by their schools;

6．推薦信。漢語(yǔ)國(guó)際教育專業(yè)碩士獎(jiǎng)學(xué)金申請(qǐng)者，須提交兩名副教授以上職稱導(dǎo)師的推薦信（用中文或英文書寫）。

6. Recommendation letter. The Master's Degree Scholarship applicants are required to submit two letters of recommendation by professors or associate professors (in Chinese or English);

7．未滿18周歲的申請(qǐng)者，須提交委托在華法定監(jiān)護(hù)人的相關(guān)法律文件。

7. Applications under the age of 18 should submit letters of approval and provide the emergency contact number for their legal guardians in China;

8．承諾書。漢語(yǔ)國(guó)際教育專業(yè)碩士獎(jiǎng)學(xué)金申請(qǐng)者須提交畢業(yè)后至少?gòu)氖?年漢語(yǔ)教學(xué)工作的承諾書（用中文書寫并簽名）。

8. Written commitment: stating that the Master's Degree Scholarship applicants will be engaged in Chinese teaching for at least 5 years after graduation (print and sign it in Chinese).

9．“漢語(yǔ)橋”世界大、中學(xué)生比賽優(yōu)勝者須提供獲獎(jiǎng)證書復(fù)印件或賽區(qū)主辦單位出具的獲獎(jiǎng)證明。

9. Award certificate copy of the "Chinese Bridge" Chinese proficiency competitions or related certifications issued by the organizers.

五、入學(xué)及年度評(píng)審

V. Approval and Annual Appraisal

1．獎(jiǎng)學(xué)金獲得者須根據(jù)接收院校規(guī)定按時(shí)到校報(bào)到。無(wú)故逾期未報(bào)到者，獎(jiǎng)學(xué)金資格不予保留。

1. The scholarship winners must register at the host institutes before the deadline set by the institutes. Scholarships will not be kept for those who do not register on time;

2．入學(xué)體檢不合格者，獎(jiǎng)學(xué)金資格將被取消。

2. Scholarships will be cancelled for those who cannot pass the physical examination;

3．學(xué)習(xí)期限1年以上的獎(jiǎng)學(xué)金生，須按照《孔子學(xué)院獎(jiǎng)學(xué)金評(píng)審辦法》接受年度評(píng)審，學(xué)習(xí)成績(jī)優(yōu)良且日常表現(xiàn)良好者方可繼續(xù)享受獎(jiǎng)學(xué)金。

3. Scholarship winners with more than one year's duration will have an annual appraisal according to the "Appraisal Procedures for Confucius Institute Scholarship" and only those who have kept good study records are entitled to continue to enjoy the scholarships.
